The first form attached is our Parent Handbook. Please read over it entirely to get a good understanding of our facility and operation policies. This book is yours to keep.
This next form is saying you have received your parent handbook. We will need you to sign, date and return it.
Number 3, this is our admission form. It is a state of Texas form and will need to be filled out entirely. Only exception is shot record portion, shot records will need to be turned in with your paperwork and you may leave this portion blank.
Next is our Discipline and Guidance form.
You will receive a packet for our food program and a media consent form, from the center during your tour, both must be returned before your child is able to be put on the door list.
Once you've downloaded and filled out your forms, please return the forms along with your child's shot records to the center between 3:30PM and 6:00PM Monday-Friday.
